Student Immunizations & Health Forms

Physical forms are required for all new students, kindergarten students, and returning sixth grade students. Blood lead screenings are required of children below the age of six if they have not been previously screened. 

All children enrolling in kindergarten and any student enrolling for the first time in school are required to have an eye examination. 

Physical & Vision Exams can be dated within 1 year of Aug. 14, 2023

All Illinois children in kindergarten, Grade 2, Grade 6, and Grade 9 are required to have an oral health examination. School dental examinations must have been completed within the 18 months prior to the May 15 deadline.
